How many breakthrough businesses start off as an answer to a question?

For Ray Kroc the question was “Why is this tiny cafe ordering so many of my milkshake mixers?” The answer was that they were busy and the reason behind this was burgers not milkshakes. Ray went on to found what is today known as McDonald’s. Takeaway?  Noticing something and noticing that you’ve noticed it are two quite different things.
